After a tentative employment offer has been made, if requested by Oxbo, I agree to take a job-related medical examination at no personal expense and authorize the examining physician to disclose the findings to Oxbo. I also understand that any offer of employment is conditional upon satisfactory completion of such job-related medical exam.
In the event of my employment with Oxbo, I understand that I may be requested now or at any subsequent time during my employment with Oxbo to submit to drug and/or alcohol tests at Oxbo’s expense. I also understand that if I refuse to take such tests, my employment may be terminated immediately.
